Once you have received your RA#, please check or write N/A, if item is not included with shipment to PID Analyzers. PID Analyzers is not liable for items left off the checklist, or for customers who do not include this checklist with their RA to PID Analyzers, LLC.

ALL UNITS, INCLUDING HARD CARRY CASE MUST BE PROPERLY PACKAGED IN BOXES WITH AT LEAST 200 lbs per inch BURSTING STRENGTH. WE WILL LEVY AN ADDITIONAL $15 HANDLING CHARGE TO OUR CUSTOMERS WHO DO NOT USE APPROVED SHIPPING BOXES. PLEASE MAKE SURE YOU INSURE YOUR UNIT FOR AT LEAST $2000.
